By making an appointment, you accept the rules and conditions of this office.
Manual Therapy and Perrin Sessions include a history of symptoms, evaluation, hands-on treatment time, and time for administrative actions such as payment, scheduling, and final questions. If you are late, you will only be treated until the end of your scheduled time. Additional time will not be added to your session, and you will be charged in full for your session. If we are running late, you will receive the full allotted time.
Medical and Mental Health Sessions are performed via Telehealth unless otherwise arranged. If you are late, your session will end at the scheduled time. If we are running late, you will receive the full allotted time.
We commit to working to the best of our abilities on that day. We will refer clients if the work required is not within our scope of practice or if the work required is outside our abilities physically, emotionally, or energetically. This may be within the clinic, or a referral outside the clinic may be necessary. We will decline to work on any client or perform specific treatments if it puts us at any risk of injury.
A 24-hour notice is required to cancel or reschedule an appointment, or you will be charged in full for the appointment. This includes traffic jams, bad weather, or sickness. Please plan your travel accordingly. Fee waivers for emergency cancellations are determined at the therapist’s discretion. Late cancellation/No-show fees must be paid by the client’s next session. If not paid by the next session, the client’s appointments will be canceled and/or the client blocked from rescheduling until paid in full.
Do not come to in-person appointments sick and/or contagious. Please reschedule before the 24-hour cancellation policy is in force, instead of waiting to see if you feel better in the morning. If symptoms did not start until the day of the appointment, then the cancellation fee will be waived.
Payment is due at the time of service. Acceptable forms of payment include cash, Venmo, credit cards, HSA, or FSA cards. All credit card charges will include an additional 3% processing fee. I do not offer direct billing, and I do not accept checks from insurance companies.
Excessive Cancellation Fee: If you frequently schedule but cancel your appointments, then an excessive cancellation fee of $100 will be applied, with the potential of being blocked from making future appointments. We understand life isn’t predictable, but please don’t abuse the 24-hour cancellation policy. Commit to what you schedule.
This office does not accept any insurance or submit documentation for reimbursement for any situation. We do not write reports for workmen’s compensation, medical insurance, or personal injury attorneys. We do not see clients that are involved in unsettled litigation, accident claims, or open insurance claims, nor do we submit statements to insurance companies related to an accident.
Loose, flexible clothing is the most effective and supportive for treatment. Shorts or joggers. Yoga pants are tight and restrictive for some of the manual therapy. Business attire or jeans are both very limiting. A bathroom is available for changing if necessary. Clothing will remain on during the entire session.
Be clean and showered on the day of your appointment.
You may have someone with you. However, please be aware that sensitive subjects come up. Be sure you are comfortable with that person knowing and hearing things. You may also have someone sit out of the room during your treatment. Minors are required to have a parent or guardian with them.
Please arrange child care for young children not being treated. Please do not bring young children who need assistance to the office to maintain a safe and effective environment.
Sexual harassment is not tolerated. A session will terminate immediately if the practitioner feels safety is compromised or a level of disrespect has been crossed.
We have back-to-back clients. As a result 1. Please do all your scheduling online to conserve time for treatments. 2. Please email any additional questions that you may have that haven’t been discussed in our treatment window.
Personal Privacy is always maintained. HIPPA standards are kept in this office. I will not discuss a client’s treatments or conditions.
Your personal contact information is only used for appointment scheduling, follow-up information regarding your appointment, or updates to our business. We will never share your information.
